Terms of Service
These Terms of Service constitute a legally binding agreement between you (the Client, passenger, or authorized representative) and Preferred Medical Transportation, LLC regarding the provision of non-emergency medical transportation and related coordination services.
1. Provision of Transportation Services
Preferred Medical Transportation, LLC agrees to provide scheduled non-emergency medical transport, wheelchair-accessible rides, and related assistance as described in your booking confirmation, written quote, or service agreement. We retain discretion over vehicle assignment, routing, and operational methods needed to perform trips safely and efficiently.
2. Scheduling, Cancellations, and Client Cooperation
Pickup and arrival times are estimates based on traffic, weather, distance, and prior commitments. Timely transport depends on accurate trip details and ready access at pickup locations. Cancellations or changes should be provided as early as possible; late cancellations or no-shows may incur fees as stated in your booking confirmation. Delays caused by incomplete information, unavailable passengers, or restricted facility access may affect scheduled windows without penalty to Preferred Medical Transportation, LLC.
3. Fees and Payment
Clients agree to pay transport fees according to the quoted rate, confirmed invoice, or executed billing schedule. Fees cover transportation and related operational services unless otherwise specified. Third-party payer requirements, insurance submissions, or facility billing arrangements do not relieve the client of responsibility for amounts not covered by a third party. Preferred Medical Transportation, LLC does not advance funds on behalf of clients for third-party reimbursements.
4. Service Scope and Termination
We provide non-emergency medical transport only. For emergencies, call 911 or local emergency services. Recurring ride programs may require minimum notice to modify or end standing schedules. Upon termination of a recurring agreement and payment of outstanding balances, we will cease future scheduled trips in accordance with the notice terms provided at booking.
5. Limitation of Liability
To the fullest extent permitted by law, Preferred Medical Transportation, LLC shall not be liable for indirect, consequential, punitive, or special damages arising from the use of our transportation services, including delays, missed appointments, or events outside our reasonable control. Our maximum aggregate liability for any claim directly related to services rendered shall not exceed the total transport fees paid by the client to Preferred Medical Transportation, LLC during the sixty-day period immediately preceding the event giving rise to the claim.
